How to Register for CKB Volunteer Service Award
Step 1:
Join the Youth Committee by filling out this form:

Note: Only 6th-12th are eligible to be a part of the Youth Committee
Step 2:
The CKB Youth Board of Directors will review your submission and send the confirmation and next steps.
Step 3:
Once you are a part of the Youth Committee, you are automatically registered for the CKB Volunteer Service Award.
Step 4:
Volunteer at CKB events and log your hours afterwards.
How to Log Hours for CKB Volunteer Service Award
Step 1:
Check with the Education Director to know the number of hours you received. Send an email to CKBEducationDirector@gmail.com
Step 2:
Fill out the google form linked below:

Download the authorization form from the link below:
Step 4:
Fill out the authorization form and get it signed by the Education Director. Send an email to CKBEducationDirector@gmail.com to get it signed digitally or get it signed physically.
Step 5:
Send the completed authorization form to CKB.youth@gmail.com
Note:
If you have registered for both CKB Volunteer Service Award and President's Volunteer Service Award, you have to fill out both google forms, but email only one authorization form.
How to Register for President's Volunteer Service Award
Eligibility Criteria:
- United States citizen or lawful permanent resident of the United States (i.e., green card holder)
- Must be at least five years old
- Completes eligible service within a 12-month period (for annual Bronze, Silver, and Gold Awards)
Age: Bronze Silver Gold
Kids (5–10 years old) 26-49 hrs 50-74 hrs 75+ hrs
Teens ( 11-15) 50-74 hrs 75-99 hrs 100+ hrs
Young Audults (16-25) 100-174 hrs 175-249 hrs 250+ hrs
Adults (26+) 100-249 hrs 250-499 hrs 500+ hrs
Step 1:
If you are interested, send an email to CKBEducationDirector@gmail.com with your name and other details.
Step 2:
The Education Director will reach out to you and verify your eligibility and if confirmed, you will be registered for the President's volunteer Service Award through CKB.

Note:
- If you have registered for both CKB Volunteer Service Award and President's Volunteer Service Award, you have to fill out both google forms, but email only one authorization form.
- You can also log volunteer hours from different organizations as long as no monetary benefits have been provided for that service. When volunteering at a different organizations, be sure to get your authorization form signed by the supervisor there. Once complete, follow step 5.
